In Beacon Hill-Cyrville, there will be a one-day clinic Saturday, November 21st, from 9 a.m to 3 p.m at the Cyrville Community Centre, 4355 Halmont Drive.
Not all children will require a second dose of the vaccine as previously recommended. The only children who need a second dose are those between six months and three years, as well as those under 10 who have chronic medical conditions. Parents bringing their children between six months and three years of age for a second dose will need to show the immunization record handed to them when the children received the first dose.
If you're sick, stay home. Getting the H1N1 vaccination when you are sick reduces its effectiveness. You can get your vaccine at a later date.
You can download and fill the following forms (also available at the clinic):
Pandemic H1N1 influenza vaccination screening tool (PDF 61 KB)
